The permanent teeth have 3 set of molars. The third set of molars is also called Wisdom teeth. They usually erupt in the oral cavity in the late teens or early twenties that’s the reason why they are called wisdom teeth.
Why are third molar impacted?
A tooth is said to be impacted when it is unable to erupt completely into a good occlusion. With evolution and eating refined diet there is very minimal inter dental wear of enamel. Hence we can many a times see that the 3rd molars don’t have enough space to erupt and often are impacted.
What problems do they cause??
1.Pain: is the most common cause that brings a patient to the dentist.
Since the tooth is unable to erupt completely it causes pain in the area. The pain can be so severe at times that it may radiate to the ear temple and jaws.
2.Cheek Biting: many a times if the molar has not erupted into the arch
and is facing towards the cheek when you chew your food there may be your cheek getting caught.
3.Crowding of Lower Front Teeth: It is controversial whether erupting 3rd molars apply
force on the other teeth and cause crowding in the lower front teeth
4.Decay: Food lodgement in the area is a common problem and maintaining a good oral hygiene in the area of the 3rd molars is difficult because of the lack of accessibility.
Many a times when patients do not get their partially erupted 3rd molars removed the not only become carious but may also pose a threat to the adjacent molar.
Surgical Technique:
1.Local anaesthesia is administered and the area numbed
2.An incision is made to expose the 3rd molar completely
3.A small trench is made in the bone around the tooth
4.Tooth is elevated either in total or may be sectioned depending on it placement and difficulty in removal
5.The area irrigated nicely and gums sutured
6.Patient is put on a course of antibiotics and pain killers
What to expect after surgery:
1.Mild to moderate pain
2.Swelling in the area
3.Restricted mouth opening
4.A small ooze of blood
All these are expected and may take 4-5 days to completely get back to normal
Instructions to the patient:
1.To bite on the cotton placed after the surgery for about 30 mins
2.Not to eat anything very hard or hot or spicy for 48 hrs
3.Do not rinse rigorously
4.Do not spit swallow your saliva
5.Do not use straw to drink anything use a cup instead
6.Do not blow any wind instruments or balloons
Always consult your dentist in case of any doubts.
